We're seeking kind-hearted and enthusiastic Health Coach Specialists to bring comfort, encouragement, and expert support to frail or elderly patients and their dedicated caregivers in Central Indiana.

Position Overview:

This rewarding remote role (with regional travel within Central Indiana) allows you to build trusting relationships while conducting home orientations, performing thoughtful patient assessments, delivering encouraging caregiver training, and ensuring every family receives the highest quality of care with dignity and respect.

Key Responsibilities:

*Lead welcoming home orientations, guiding families through paperwork and initial assessments with patience and clarity

*Conduct bi-monthly supervisory visits and caring check-ins to offer ongoing support and reassurance

*Provide empathetic caregiver coaching, training, and encouragement to help them thrive in their vital role

*Build positive connections with patients, caregivers, and stakeholders

*Collaborate in interdisciplinary meetings with Area Agencies on Aging (AAA) and Managed Care Entities (MCE) Care Managers

*Contribute ideas to care plans and process improvements with a solutions-focused, can-do spirit

*Adapt with flexibility and optimism to meet evolving needs

*Travel regionally throughout Central Indiana (valid driver's license and insured vehicle required)

Who We're Looking For:

We're searching for empathetic individuals who naturally radiate positivity and truly enjoy making others feel valued and supported.

Ideal candidates bring:

2+ years of experience working with frail, elderly, or disabled adults and their caregivers

Field experience in medical, case management, or home care settings (preferred)

Knowledge of ADL/IADL assessments

Experience with data entry and case notes (Salesforce a plus)

Outstanding communication skills and a genuine ability to listen and uplift others

Strong organizational skills, critical thinking, and a collaborative team-player mindset

Excellent training abilities and a passion for empowering caregivers

Proficiency in Microsoft Office (Word, Excel, Outlook)
