As a leading provider of assisted living, skilled nursing, and rehabilitation services throughout the Western United States, we are dedicated to empowering residents to achieve their maximum independence and enhance their quality of life.

Why You’ll Enjoy Being Part of Our Team

  • Competitive Compensation & Benefits: We offer hourly or per-visit pay based on experience, along with medical, dental, and vision coverage, generous Paid Time Off, holidays, a 401(k) plan, and more.

The Position

In the role of Physical Therapist (PT), you will assess the physical capabilities of residents and create tailored treatment plans aimed at helping them regain or sustain mobility, functionality, and independence. Your responsibilities will include providing hands-on therapy, educating patients and their families, and working with interdisciplinary teams to achieve the best possible outcomes.

Key Responsibilities

  • Perform thorough assessments of residents’ physical abilities and mobility using standardized assessment tools.
  • Create and execute evidence-based treatment plans to enhance strength, balance, coordination, and range of motion.
  • Deliver therapeutic interventions, including exercise regimens, gait training, and the use of assistive devices.
  • Instruct residents, families, and staff on safe mobility practices and home exercise routines.
  • Accurately document evaluations, progress notes, and discharge summaries in a timely manner, adhering to regulatory and organizational standards.
  • Collaborate with occupational therapists, speech-language pathologists, nurses, and other team members to provide integrated care.
  • Ensure compliance with all relevant state and federal regulations.

Qualifications

  • Active licensure as a Physical Therapist (PT) in the state where you practice.
  • Graduation from an accredited Physical Therapy program.
  • Preferred experience in long-term care, skilled nursing, or rehabilitation settings.
  • Excellent communication, documentation, and organizational abilities.
  • A genuine compassion and dedication to enhancing residents’ quality of life.

Join Our Team

If you are eager to apply your skills and compassion in a mission-focused organization that values both residents and employees, we encourage you to apply and advance your career with us.

We are an equal opportunity employer and do not tolerate discrimination or harassment based on race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state, or local laws.

To maintain a safe environment for all employees, residents, families, and visitors, we strongly encourage our team members to receive an FDA-approved COVID-19 vaccination, along with any recommended booster doses from the Centers for Disease Control and Prevention (CDC). To support this initiative, we occasionally host onsite vaccination clinics at our various locations. However, being fully vaccinated for COVID-19 is not a requirement for employment.
