Job Summary

We are seeking a dedicated and skilled Nurse Practitioner to join a growing team of healthcare professionals. The ideal candidate will provide advanced, comprehensive primary or specialty care to patients in a clinical setting. Responsibilities include performing physical exams, documenting medical histories, developing care plans, ordering diagnostic tests, and promoting health and wellness.

Key Responsibilities

• Conduct physical examinations and patient assessments
• Diagnose and treat acute, chronic, and preventive conditions across various age groups
• Maintain accurate and thorough documentation including medical history, review of systems, medications, and care plans
• Order and interpret diagnostic tests including lab work and imaging
• Develop and implement individualized treatment plans
• Prescribe medications and therapies in accordance with state and federal regulations
• Provide patient education on disease prevention, health maintenance, and treatment options
• Collaborate with specialists and refer patients when necessary
• Ensure a high standard of patient care and support quality improvement initiatives
• Administer immunizations and screenings as needed
• Participate in team meetings and contribute to continuous patient care initiatives

Qualifications

• Master’s Degree from an accredited Nurse Practitioner program (Family Nurse Practitioner or Adult-Geriatric Nurse Practitioner)
• Active New York State Nurse Practitioner license
• National board certification (e.g., AANP , ANCC , or ONCC )
• Strong clinical judgment and ability to work independently and collaboratively
• Bilingual (Spanish-speaking) candidates are a plus
• Experience with EMR/EHR systems preferred
