This is an Open Competitive Career Service Senior Community Health Nurse position working in the Lee County Health Department.

This Senior Community Health Nurse is responsible for planning, monitoring, evaluating, and providing services on a countywide basis in outreach immunization clinics, employee health, and the health department. This position is under the direct supervision of the Nursing Program Specialist.

Assists with Immunization Program activities/services for the agency according to the current edition of the Epidemiology and Prevention of Vaccine-Preventable Diseases (CDC’s The Pink Book), the ACIP Recommendations , and guidelines from the State of Florida Immunization Program.

• Promotes and maintains health of individuals, families, and the community through appropriate measures.
• Provides comprehensive nursing; vaccine administration in all FDOH- Lee clinics and community settings.

• Adheres to Immunization Program Guidelines; correct use of VFC (PC01), Uninsured/Underinsured (PC17), purchased vaccines (PC05), and Hepatitis (PC09) vaccines.
• Assures proper documentation of immunization services into Florida SHOTS and HMS.
• Maintains proper storage and handling of all vaccines, as per recommendations of the current edition of the CDC Vaccine Storage and Handling Guide.
• Utilizes monthly reports (Reminder Recall/Televox) to send reminders to clients who are coming due/past due for vaccinations.
• Administers vaccines for overseas travelers and maintain up-to date overseas travel information and is knowledgeable of the appropriate vaccines to administer relevant to the area of travel. Maintains current references for childhood, adult, and overseas travel immunizations.
• Provides statistical reporting as required and/or requested.
• Communicates anticipated vaccine and supply needs with Immunizations supervisor, monthly and/or as needed.

Serves as subject matter expert and resource person for Immunizations and Employee Health, providing technical assistance and educational support to nursing staff at DOH-Lee, and as needed to private providers and area CHDs.

• Maintains complete and current knowledge of all aspects of the immunization program including, but not limited to; childhood (Vaccines for Children-VFC), adult, travel, and providing outreach services.
• Conducts workshops and sends educational CEU opportunities to staff.

• Performs immunizations skills checkoffs for clinical staff.

• Promotes access to vaccines in the community for adults, children and special needs/high risk populations through local coalitions and other community partners.
• Reviews DOH-Lee Employee Health protocol, as necessary. This protocol includes health promotion (wellness) and health protection (immunizations, TB, occupational health, and safety) components.
• Attends pertinent conferences, meetings, and events as requested or permitted.
• Responds to public inquiries regarding immunization guidelines and recommendations in collaboration with DOH-Lee’s Public Information Officer (PIO).
• Responds to DOH employee inquiries regarding Employee Health policies and recommendations.

Plans, coordinates, and monitors outreach programs to ensure efficiency and benefit to the community.

• Assists with the outreach immunization program for children and adults as needed, which could include evening or weekend hours.
• Aligns outreach initiatives with the county health department, health care providers, health management organizations, senior centers, active coalitions, nursing homes, and assisted care facilities.
• Conducts educational presentations and implements immunizations campaigns to increase community awareness.
• Completes and submits monthly outreach documentation; DOH Lee Meetings and Hosted Events.
• In conjunction with the Public Information Officer, maintains and distributes current list of outreach clinic events and sites to appropriate persons and agencies, conducts educational presentations to enhance awareness while reinforcing the importance of vaccination, c reates handouts related to disease specific conditions for citizens and for the medical community.
• Assists with health fairs, community organizations, and specific public health events, as requested.
• Performs quality improvement activities to monitor the effectiveness of outreach activities.

Performs Employee Health nursing duties as outlined in FDOH Lee IOP 13-3. This includes but is not limited to:

• Conducts Employee Health clinic for all new hires and completes necessary paperwork.
• Reviews and maintains employee health records.
• Educates employees on mandatory and recommended immunizations.
• Administers employee immunizations and/or facilitating acquisition of necessary titers.
• Assists with coordination of needle stick exposures and post exposure prophylaxis.
• Advises employees about job-related health risks and exposures.
• Coordinates employee flu clinics at all FDOH-Lee sites annually and other vaccines, as necessary.
• Assists the Public Health Emergency Preparedness Program with fit testing with N-95 masks.
• Conducts TB risk assessments annually or as needed according to the employee’s risk of contracting TB.

• Updates employee spreadsheet (notes whether staff is new, current, or separated) and immunizations provided, monthly.

Monitors climate-controlled storage (Verilert) for vaccines by responding to alerts and alarms in a timely manner and ensuring proper temperature logs.

• Responds to special directions and instructions during emergency operations, secondary person responsible for the Vaccine Storage Emergency Response Plan.

Timely and accurate documentation of leave and attendance records; biweekly timesheet and daily employee activity records (EARS).

Assists with student and volunteer orientation by precepting and evaluating students through observing their performance in various settings, monitoring their work, and providing input to students’ clinical coordinator.

Performs other duties as assigned.

Licensure/registration/certification requirements (If applicable, list the appropriate Florida Statute or federal regulation cite):

• Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.

Other job-related requirements for this position :

• Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to threats involving any disaster or threat of disaster, man-made or natural.
• Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.
• Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.
• This position will abide by all state and federal laws, rules, and DOH policies and procedures .
• This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.
• Must have current CPR with AED (BLS) certification or obtain such certification during employment.
• Must be willing to undergo FIT Testing.
• Successful completion of a drug test is a condition of employment.
• This position is classified as sensitive. This individual will perform tasks defined as sensitive and handles confidential information. Background screening is a condition of employment.
