Job Title: Homeless Outreach Specialist – Enhanced Care Management - Bilingual
Reports to: Operations/Program Managers
FLSA Status: Full time, Non-Exempt, Hourly (DOE) Schedule: Monday - Friday 8 am – 4:30 pm
Compensation: $25 - $28 per hour , commensurate with experience. Holiday, vacation, and sick pay .
The mission of Doctors Without Walls-Santa Barbara Street Medicine (DWW) is to provide free, volunteer medical care for the most vulnerable, underserved populations in Santa Barbara County, when and where they are in need, including in times of disaster and to provide education and training for DWW volunteers and others, in order to promote the excellent practice of humanitarian medicine in our own community.

Position Objective: The Homeless Outreach Specialist supports homeless individuals and families residing in South Santa Barbara County. The position is case management “on-wheels” and liaison to community outreach partners.
Position Summary
Under the direction of the Operations/Program Managers, the Homeless Outreach Specialist will act as an advocate for individuals experiencing homelessness in South Santa Barbara County. The Homeless Outreach Specialist will assist clients with navigating additional care systems, including medical, housing, homeless certification, connections to various service providers. This position will have an emphasis on Doctors Without Walls Spanish speaking population. The Homeless Outreach Specialist will work in partnership with Doctors without Walls clinical staff, behavioral health providers, social service agencies, and various levels of law enforcement. The Homeless Outreach Specialist will assist and provide clients with resource guides, and information about accessing services and other benefits unique to the homeless population through Street Case Management & Housing Navigation.
Essential Job Functions & Duties
• Perform outreach services to the homeless with a concentration on Spanish speaking clients including the distribution of meals, clothing, hygiene supplies.
• Identify needs and provide resources for Medi-Cal, mental health, substance recovery, educational programs, financial and housing assistance services.
• Develop good working relationships with government and community resources such as business owners, realtors, housing developers, etc.
• Maintain client-related data tracking systems including case notes in Electronic Medical Record (EMR).
• Attend collaborative meetings such as staff training and weekly case conference.
• Transport clients to appropriate services, as needed.
• Schedule and facilitate outreach activities for individuals experiencing homelessness on the streets of South Santa Barbara County 220 West Canon Perdido, Suite 3, Santa Barbara, California 93101 805-455-4234
• Meet and assist clients daily, via phone or in person, with medical care, substance abuse treatment, mental health counseling, and housing opportunities
• Document all contacts and outcomes in the Electronic Medical Record, uploading necessary information to an external database on a case-by-case basis
• Adhere to all local, state, and federal guidelines and laws I.e. HIPAA, OSHA, FLSA
• Maintain and develop relationships with federal, state, and local agencies that serve as referral resources for clients
• Perform special assignments to this unique function (homeless counts, data processing, presentations)
• Other duties, as relevant and assigned by Program Managers and Executive
Skills & Abilities:
• Adept in MS Office (Excel, Word, and Google).
• Skilled at working with diverse populations and engaging clients.
• Ability to communicate professionally and with integrity.
• Ability to work independently or as a member of a team.
• Excellent problem-solving skills.
Knowledge:
• High School Diploma or GED (AA or BA preferred)
• Bilingual (Required)
• Valid Driver’s License required
• Experience with the homeless population.
• Experience in street outreach and/or the homeless shelter environment.
• Knowledge of local resources for the homeless and underserved populations.
