Private Duty Nurse, LVN - Pediatrics at At Home Healthcare summary:

The Licensed Vocational Nurse (LVN) provides high-quality, patient-centered nursing care to pediatric patients in home healthcare settings. They collaborate with registered nurses and care teams to implement personalized care plans, document care in electronic medical records, and support patients in achieving their health goals. This role requires compassion, independence, and accountability to deliver skilled nursing services while fostering a supportive and flexible work environment.

The Licensed Vocational Nurse works to ensure high-quality medical care and support for children with different healthcare needs. Our Licensed Vocational Nurse approaches each day with a passionate pursuit of caring for patients by working one-on-one with them and helping them to achieve their health goals.

As the Licensed Nurse, you will:

• Work in collaboration with the RN to fulfill the defined patient-specific care plan
• Collaborate with the care team to ensure all patient needs are fully addressed
• Deliver high-quality skilled care to patients
• Thoroughly document care delivery daily in our EMR system
• Contribute to a culture of caring through individual accountability and teamwork

• Able to work independently without direct oversight
• Able to discern when to call for support and communicate challenges
• Familiar and comfortable with technology. HCHB experience is a plus

Experience to Deliver on our Mission:

• Current Vocational/Practical Nurse License, valid state driver’s license and auto liability insurance
• One year experience as a Licensed Vocational/Practical Nurse in a clinical care setting required. Home health experience preferred
